Hello, my name is Leonard Lado MD^TM and welcome to my practice in Bonita Springs Florida we are centrally located between Fort Myers and Naples Florida . right of Tamiami trail. I guess you have come to seek answers to psychological and psychiatric problems afflicting you. I will do my best to answer those questions you may have. Perhaps you have been referred by the insurance company, primary care physician, friend or self.  I welcome most if not all psychiatric evaluations.

The initial evaluation will allow me to determine what the best treatment course for your problems is. The recommendation may include an outside referral if the problem is beyond the scope of my practice or the severity of the condition that requires frequent crisis intervention. Currently, I am not affiliated with any inpatient psychiatric facility. However, if you have a very severe substance abuse problem that is out of control, making you loose your family, involving DUI and other legal problems,  then admission to The Willough at Naples may be necessary.

I try to keep the practice simple and personal but as you can see from the paper work involved, federal and state regulations mandate a huge number of forms and signatures to protect your privacy. Just complete the paper work and don't get too frustrated. You probably have enough problems before I evaluate you. The paper work does have to be completed for me to render any care. We need to get some administration points out the way to best serve your needs and allowing to me spend more time with you.

1. WE DO NOT PERFORM Telephone Refills. You need to keep your scheduled appontments. Our prescriptions are completely electronic via E-Prescribe. This means that prescription are usually ready by the time yu arrive to the pharmacy. It also means access to pharmacy records to prevent significant drug interactions. We alo have a dedicated phone extension just for pharmacy at (239) 948-4325 to clarify questions only.
   
   
2. WE DO NOT PRESCRIBE methadone, alprazolam (Xanax) , Hydrocodone (percocets), diazepam, oxycodone or amphetamine stimulants. As I mentioned  in this webpage Attention Deficit Hyperactivity disorder, I diagnose and treat ADHD after all other illnesses have be subdued and stabilized.
   
   
3. If you are having a life and death crisis please call 911 as soon as possible and then call me to see what else I can to help. If you are having a drug reaction please call me so I can intervene.
   
   
4. We now acccept certain insurance companies such as AETNA, CIGNA , Humana Military Health Plans , ORIGINAL MEDICARE , REGULAR MEDICAID  and FLORIDA HEALTH SOLUTIONS. Other insurance plans are soon to come. Please make sure that you notify your insurance company of your intentions to make an appointment in our office. This is usually required by some some insurance companies in order for the visit to be covered. Deductible and Co-Payments must be paid a the time of visit. I will be able to spend better time taking care of you than worrying about how to pay the bills for the office. Please remember that it is your ultimate responsibility for the bill. As a courtesy to you with accept insurance. However, the ultimate responsiblity is yours only. Therefore please make sure you give us you correct insurance card as insurance companies often update you with new cards. These cards often have different ID numbers and therefore without the correct ID number, your payment will be denied to us. Since it is your responsibility for you to know what you are paying for insurance, Please complete the insurance verification form prior to coming to the office
    
   
5. MISSED APPOINTMENTS not cancelled within 24 hours will be charged $50. This fee is not reimbursable by the insurance companies and must be paid at the time of the next office visit. Medicare now permits physicians to charge for missed appointments. We will verify appointments by telephone and/or email 24 hours before your visit. If we do not hear from you within the 24 hours you will be charged $50. There are no exceptions to this rule.
   
   
6. I do not perform supportive or any other type of psychotherapy over the phone. Please bring them to the office. The telephone does no justice in caring for your problem.
   
   
7. Please be kind to the staff that is making your appointment. They are trying very hard to help you with your concerns. Please dont yell at them if I am spending extra time on patient that needs extra help and is ahead of you.
   
   
8. Request for medical records are handle via fax or e-mail only. Our system goes thru a HIPAA compliant server to protect your records. We have an electronic record system that speeds up the process of sending your records confidentially by fax and email. You must sign a consent form before we can send any records or information. The fee for request for medical records is $25.
   
   
9. The completion of disability, school or work related forms/documents has to be done in person and billed as an office visit. You have to be present because many questioned asked requires your presence. These evaluations are not covered by insurance companies and require other elements in your condition with tedious paperwork.
   
   
10. I will always require a copy of your drivers license to assert your identity as part of HIPPA regulations. You need your updated insurance card for us to call and help you pay for your treatment.
    
    
11. Please be aware if you are court ordered or attorney referred for treatment or evaluation that your medical records may not be protected by HIPAA laws and may be public records and you waive your privacy rights. You must fill out the consent form so we can discuss your case with your attorney. COURT ORDERED evaluations and disability evaluations are NOT COVERED by insurance companies and you will need to pay. A retainer will be required with all ATTORNEY related cases.
    
    
12. SUBOXONE TREATMENT IS NOT COVERED BY MOST INSURANCE companies including medicare please inform yourself of coverage prior to making an appointment for this treatment.
    
    
13. INTEGRITY AND TRUST is an important part of your recovery. If you are not honest with me then I cannot help you. Recovery of your illness requires that you are honest with your self first and avoid ingratiating others as it will lead to self deception and eventually your own demise and failure of treatment.
